Welcome to MyHIN.org. This site is owned by the Divide Classic, a non-profit organization founded in 1998 to assist families and individuals living with hydrocephalus. The MyHIN Personal Health Record was created so that individuals diagnosed with hydrocephalus can have access to their important health information at any time. Information Collection and Use

MyHIN does not collect, rent or sell your personally identifiable information. This includes but is not limited to mailing addresses, telephone or fax numbers and email addresses. Your contact information is stored by MyHIN in a safe and secure manner.

My HIN may offer users, from time to time, the opportunity to register for surveys and clinical trials and other research related activities. We will do this by requesting that users complete a form authorizing the release of contact information.

MyHIN adheres to the principles and guidelines of the Health Insurance Portability and Accountability Act (HIPAA). The privacy provisions of the federal law, the Health Insurance Portability and Accountability Act of 1996 apply to health information created or maintained by health care providers who engage in certain electronic transactions, health plans and health care clearinghouses. The Department of Health & Human Services (DHHS) has issued the regulation, Standards for Privacy of Individually Identifiable Health Information" applicable to entities covered by HIPAA. The Office for Civil Rights (OCR) is the Department component responsible for implementing and enforcing the privacy regulation.

The information stored on MyHIN belongs to the individual subscriber. Only by permission of the user/subscriber may information, including passwords, be shared with other parties.

MyHIN is not a "covered entity" as currently identified by HIPAA regulations. However, MyHIN strives to maintain the privacy and security of Personal Identifiable Information as if MyHIN was covered entity under the federal HIPAA guidelines.

What is Protected Health Information?

Any individually identifiable health information, oral or recorded, in any form that is created or received by a provider, health plan, employer, clearinghouse; and relates to a past, present or future mental or physical condition, the provision of health care to an individual, or the payment for the provision of health care to an individual.

Security

MyHIN takes every precaution to protect users' information. When users submit sensitive information via the website, your information is protected both online and off-line.

When our registration form asks users to enter sensitive information (such as a credit card number and/or social security number) that information is encrypted and is protected with the best encryption software in the industry - SSL. While on a secure page, such as our order form or your medical record forms, the lock icon on the bottom of Web Browsers such as Netscape Navigator and Microsoft Internet Explorer becomes "locked", as opposed to un-locked, or open, when you are just surfing. While we use SSL encryption to protect sensitive information online, we also do everything in our power to protect user-information off-line. The servers we use to store personally identifiable information are located in a professionally managed co-location facility with state of the art security measures.

Notification of Changes

If we decide to change our privacy policy, we will post those changes on our homepage so that our users are always aware of what information we collect and how we use it.
